As a courtesy, we will bill insurance directly if your policy includes massage and acupuncture benefits. Our clinic is In Network as a Preferred Provider with the following insurance panels and you may schedule with any provider:

Benefit Check
Fill out the form below or call us with your insurance information prior to your appointment so can verify your benefits. We will need to see your insurance card along with valid ID at your first appointment.
Medical Massage
Medical Massage is billed through your insurance in 15 minutes increments at a rate of up to $40.00 per unit. This includes actual time covering the treatment on credit, billing hours, tracking, collecting payment, documentation and chart notes. Reimbursement is dependent on eligibility of services at time of service and all terms and conditions of your plan.
Scope of practice
Licensed Massage Therapists are only allowed to bill under our scope of practice. These codes include CPT 97124 (Therapeutic Massage) and CPT 97140 (Manual Therapy)
Patient Responsibility
It is patient’s responsibility to monitor your insurance benefits. As a courtesy, Elk Rock Wellness will bill your insurance on your behalf. It is your responsibility to read your own policy, know your coverage, and review the explanation of benefits statements regarding payments. Please remember your insurance policy is a contract between you and the insurance company, we cannot guarantee payment of your claims It is ultimately your responsibility to pay for any balance not paid or covered by insurance. Any charges that are not covered by the given insurance company will be billed to you.
